MySQL SQL PHP

Description

Hello All-
I have a working, developed form in PHP. PST if you'd like addy.

This form gathers information from the user on their debt accounts. It then presents the user with a quote.

I would like a few enhancements to this form.

1. I have the .sql containing the db structure as well as CSV files containing the data. This simply needs to be imported onto my server via phpmyadmin.
2. When user enters their creditor account number (and each user can have multiple accounts), I would like our database check based on the format for the number they entered. For example, if they enter 423849248892, the database contains a column of account number character lengths and account number masks, so you would match up the DB to find a creditor ID that contains the right amount of digits and then check the other column to find the creditor within those with the right # of digits to find the right mask. This will evolve creditor IDs. You will then reference another table to find the CREDITOR NAMES associated with these CREDITOR IDs. At this point, a drop down list (because there may be multiple results with this criteria) would be populated with the creditor name. If the account number doesn't match a specified match, the drop down list should be populated with "Creditor Not Found"
3. When user selects a creditor, you will perform two more database checks. Reference the particular creditor they chose and see if eliminateInterest=1 was set in the database. If it was, then set the APR/interest for that particular creditor to 0 in the calculations to evolve the quote. If it is not checked, reference the column titled reduceInterest. If this was set to 1, reference the interestRate column and set the APR/interest rate in the application/calculation to the associated value found in the interestRate column. If neither columns are set to 1, then leave the APR as is. As of right now, the form calculates the final interest rate as the user's entry -5. To get a more accurate number, we need to do this db check and switch that -5 value to whatever the checks from the database tell us to do.

In summation, your task consists of the following:
Import a few tables via phpmyadmin
Establish a new database connection in PHP in a form that already has a database connection
Populate a list based on user entered criteria as compared with a MySQL database
Set a value based on a MySQL database

Would like this done in 3 days, max!
