Looking to create a incident report form using PHP scripting. &#160;Must dump data to a 2005 Microsoft Sql server.&#160; The database is configured and has two tables one table has 61 columns and the other is 8 colums.&#160; The form will ask the user for Name, address, etc.&#160; Would like the form to hide and show data when an item is selected from an drop down menu.&#160;&#160; No checkboxes or radio buttons are needed. &#160;For example if the person injured is a employee it will show a set of certain fields and if they are a visitor or patient it will show a defined set of form fields.&#160; Also will need the capability to using multiple fields.&#160; For example if they need to add multiple witnesses or individuals.&#160; Form validation is required, like date &amp; time and phone number validaton.&#160; NO email validation.&#160;&#160; I can send you the form in html for basic form layout.<br/><br/>Thanks,<br/><br/>PIno<br/>
