I am looking for a web designer/ programmer who will create multiple answer inquiry form for an existing ecommerce webpage. The entire form will consist of 6 questions with 5 choice answers for each question. The authenticated User will be able to choose an answer by selecting appropriate radio button before continue with check out. The script will be created either in java or php and then will be stored on mySQL database by clicking a submit button. The user-end design needs to be simple and clean no graphic artists needed. Please have verifiable portfolio of similar work.
