The entire project will contain 4 pages survey-consultations intended to collect information from an online user and save in a database table.
The desired candidate will demonstrate proficiency in java and php technology and will be responsible for creating animated slide sheets using Java or Ajax. Each of the animated slides will be displayed on one form and will show survey questions and provide choice of multiple answers by the use of radio buttons.
When the user answers all questions the feedback will be save in a Apache server in a table. Conformation, with the database print for the each transaction will be sent via email and received by provided recipient.
Project DOES NOT requires advanced graphics but some degree of esthetics and composition is expected.
Candidate must have excellent english communication skills.
