The app will be very simple.

1)   image appears while app is loading (image to be provided)

2)  INTRO page- contains some static text (to be provided)

3)  FREE SAMPLES page-  Static page of text with a few links- (content to be provided

4)CONTACTS page... page listing about 100 phone numbers and some other data for each record- I will provide these records in a spreadsheet

A few of these records will not be phone numbers- but links- instead of phone number- the URL appears- click opens up website in iphone browser

Section is a list of phone numbers and URLs.  It is a static list I will provide you in a spreadsheet or txt whatever is easier . It will be like this

[number or URL]
 [id]

Each record should look like this -
Formula 
Free Similac, Isomil or Other Formulas, & Coupons

The user can click to call or click to visit url one or the other not both

There will be 2 buttons for each record
|Contacted|- (If the user clicks "Contacted" the display for this record becomes transparent but  grayed out- but still readable to indicate they have contacted this record. the user can click "contacted" again to restore to initial state

Report an Issue- opens users email program on iphone- Subject line becomes - Report an issue with phone number [ID] # 00124

On the top and bottom of this page - there is a button
|Report new number| - User can click it opens users iphone email program- subject line defaults to " Report a New Phone Number"

So 3 pages total:

 INTRO 
 FREE SAMPLES
 CONTACTS 

Also need an icon for each page to appear as the navigation within the app- Im not picky at all- 

contacts could be a phone icon
free samples could be a baby diaper icon
into could be --- hmm some generic icon

Please provide an estimate for this work. 
This is not my first iphone app- I know this is a very simple job- if you do good work- I may use you again
