I'm in charge of making a survey for a group of people and I want it on the iPhone. The survey is for a song I made. I'd like to keep it cheap, so I tried to limit the functions in the app. Nonetheless, I think it'd be fun and cool for my friends. I'd like my testers to be able to go onto the app store and download it.

Here is what I'd like it to have:
-A page of basic information, my contact information, and why I'm doing the survey.
-A preliminary survey that they must fill out (age? race? where were you born? what city are in you in now, etc.).
-A way for them to play my song within the App. Each time they play the song, I'd like to record the time and date. Ideally, they should be able to grab their iPhone, unlock it, click my App on the home screen, and immediately be able to "Play" my song (no need for them to go to an informational page every time)
-Since I'm studying this, I'm asking my friends to listen to the song 20 times per day. So I'd like a countdown so they know how many more times they need to listen to it. They are welcome to play it more than 20 times, but after 20 times you can stop recording the number of times they play it.
-They will do this for 10 days in a row.
-At the end of 10 days, they must fill out a post-survey (how did you like it? did you get bored of the song? did it work? etc.).
-Then they can submit the information to me.
-I'd like all the data from a given tester (her pre-survey responses, her post survey responses, and the times and dates she listened to my song) all in one Excel spreadsheet in one vertical column.

Also, after this has been accepted to the iPhone app store, I'd like to update it. I'd like for them to take a picture of themselves with the iPhone camera during the preliminary survey. Then I'd like for them to take a picture of themselves during the Post-survey. When they submit their data, then the pictures come along with it!

File: "Consent form liability to home.pdf"
-When iBbbbb is opened for the first time, there will be a liability disclaimer form that pops up. "The developers and researchers are not liable for any problems this application may cause." At the bottom, please give the users the options to "ACCEPT THE AGREEMENT" or "NO I DO NOT AGREE".
-If user do not agree, please have the application quit.
-If user agree, let the application open to the homepage.

File: home to instructions to survey.pdf
-The first page in the PDF file is the homescreen.
-The homescreen has a top button "Click here after you have completed the pre-survey and want to start data collection." I will explain this later in my email.
-The homescreen has a PLAY button so the user can play my 1 sound. Each time the user presses the PLAY button, the song plays. The user may play my sound as much as he wants.
-The homescreen has an INSTRUCTIONS button. The instructions are on page 2 of this PDF.
-The homescreen has a SURVEY button. The survey screen is on page 3 of this pdf and user has choice of pre survey (show on page 4) or a post survey.
-The homescreen has an ABOUT button for information about your company and my company. "This app was developed by Cherry Wang and Michael Fujinaka email is michael.xxxxx@gmail.com"

-After the user has completed PRE-Survey, user can click the top button on the homescreen. The top button says: "Click here after you have completed the pre-survey and want to start data collection."
-When this top button is pressed, the countdown starts.
-Countdown starts with " Day 1. remaining plays: 20"
-Each time user presses PLAY button, the count goes down, "Day 1. Remaining plays: 19". Also, each time user presses PLAY button, program records the date and time the sound was played.
-When user presses PLAY, the count goes down, "Day 1. Remaining plays 18."
-When user presses PLAY, the count goes down, "Day 1. Remaining plays 17."
-When user presses PLAY, the count goes down, "Day 1. Remaining plays 16."
-When user presses PLAY, the count goes down, "Day 1. Remaining plays 15."
-If user presses PLAY button 20 times, the count down stops at 0. "Day 1. Remaining plays 0." User may still press play and program records date and time sound was played.

-At 11:59pm the DAY increases by 1 day and count down resets. So it would be, "Day 2. Remaining plays 20."
-When user presses PLAY, the count goes down, "Day 2. Remaining plays 19."
-When user presses PLAY, the count goes down, "Day 2. Remaining plays 18."
-When user presses PLAY, the count goes down, "Day 2. Remaining plays 17."

-After 11:59pm on day 10, top should read: "Data collection finished, please complete Post-Survey." The user may still press play to listen to the sound. Program DOES NOT record date and time after 10 days.

-After the user has taken the survey for 10 days in a row, the user is able to take the POST SURVEY. After the user completes the POST survey, user can SUBMIT DATA. This emails the responses to me (michael.fujiXXX@gmail.com) in Microsoft Excel vertical column. I have attached an example of how i want the survey emailed to me. There should be 2 columns: one column for the pre-survey and one column for the post-survey.
Ok, let me know if you have any questions! Oh, I do not think i will be buying camera function. I emailed Apple.com and they said that this application cannot use a camera. So I do not need a camera.
