I would like to post the project here again and search for a more suitable developer.

The application is to server as a booking and confirmation application.

User enters in details in to the application and the details are send as plain text to a recipient email address.

Source code is needed for compiling here and submission to the apps store under our company name.

Successful applicant must have completed other iPhone applications. Turn around time is approximately 2 weeks from project acceptance. I've attached some graphics for your reference.

Detailed Description:
1) the loading screen will appear temporarily as content and images are loading in the app and continue to the terms page automatically.

2) Terms will appear and require the user to click start booking. Might need a clause that states, "by clicking Start you there by accept these terms and conditions show here and detailed on our website." or something to that affect.

3) So each feild when clicked will bring up the type pad input for users to add in their details. e.g. name, address, email, phone. . These details will appear on next to the field names after the user has submitted data.

4) preferred time will give them the options as well as preferred days.
Options include: Mon, Tue, Wed, Thurs, Fri
Morning, Afternoon

5) Add to List will bring up a list of items that the user can select from. The list of approved items that you accept and add them like a shopping list. User can scroll up and down the list and click to have an item selected.

6) user then will using the slide action button to select if there will be help at home.

7) finally the submit
All the data will be then sent to the collections email address. and the user is presented with a thank you screen and a link to the diabettes website.
