What I need or require: Convert paper forms to web based forms.

What I already have versus what the provider will deliver: I have the definition of the form. I would like to see how these can be converted to the web format and improve on the paper format. The improvement will involve
1. Automated data validation and verification check
2. Usability improvements so that the user is encouraged to fill up the form and provide maximum input.

Other context/requirements that providers will need to know: I have to fold these web forms into ASP.Net/C# application. So the form should be ready for that, preferably in a ASCX format. I would like to see if someone can also jump start the forms with JQuery based Ajax functionality to make the forms dynamic. The forms must be CSS driven.

Specific expertise/background that I am seeking:
Good Graphics design. Good understanding of Usability issues. Familiarity with Asp.net framework. Good knowledge of jQuery. There is not need to do a Business logic or a data base coding.
