iPad App for Information & Liability Release then Export PDF to Server (80% Done)

I need an iPad app that will help us reduce our paperwork load.

We run a company that the customers have to waive any liability etc. There are two forms for them to sign and answer yes/no or agree to various questions.

I have them in PDF format, however I want to reduce paper waste and I want to create this form for the iPad.

Here is what I envision.

The user opens the app.

There will be a home screen with buttons.

The sign now button will bring you to a list of various questions for yes & no. Then the user will sign the screen that him or her agree. This will then produce a pdf that will be uploaded to our server as a file. Then there is a second document for the same thing and then upload to server again.

Lastly I will need some type of screen that only I can get into that will show me all the past uploaded forms sorted by date so I can make sure they were uploaded. I will also need to be able to pull them up on the ipad if needed from the server.
